What is the Commonwealth Seniors Health Card, and am I eligible?

The Commonwealth Seniors Health Card (CSHC) is a government-issued card that provides those of Age Pension age with access to a wide range of health concessions and discounts. These concessions include assistance with prescription medication, dental and eye care and various other health related matters. It may also provide discounts on public transport fares and discounts on utilities.
The card is designed for those who are of Age Pension Age, who are not eligible for an Age Pension due to the Income or Asset Test. Basic conditions include:
Being of Age Pension Age - this varies, depending on your date of birth.
Meeting Residency Requirements - typically an Australian Citizen or permanent visa holder, living in Australia.
Not be getting an income support payment from Centrelink or the Department of Veterans' Affairs.
Provide Services Australia (Centrelink) with your Tax File Number and identity documents.
Have income under the Income Test.
The Income limit for the Commonwealth Seniors Health Card has recently been increased. To meet the Income Test, you must earn less than the following:
$90,000 if you're single
$144,000 a year for couples
$180,000 a year for couples separated by illness
There is no asset test.

As with all Centrelink benefits, the Income Test can be complex and is based on your adjusted taxable income and 'deemed' income (rather than the actual income received from an Account Based Pension).
